Problem on OPCDA data source module

I downloaded the OPCDA data source module 1.5.1 for MA2.1, and loaded the module successfully in mango.
But when I selected the data source and clicked to create an OPCDA data source, nothing displayed under the data source tab on the page.
what's the problem? thanks for reply.
